Arch Expander in Ottawa: Addressing Jaw Width and Dental Alignment

Traditional palatal expander

If your child’s teeth appear crowded—or if your own bite is misaligned—the concern may relate to the width of the upper jaw rather than the teeth themselves. At The Teal Umbrella, we utilize arch expansion to address jaw width. This clinical protocol is designed to widen the upper arch to create the necessary space for tooth alignment, support airway volume, and establish a foundation for improved bite function.

What is a Palatal Expander (Arch Expander)?

An arch expander is an orthodontic appliance used to widen the upper jaw over a period of several months. This palate expander fits across the roof of the mouth and attaches to the upper molars. In children and teens, the two halves of the upper jaw are connected by a palatal suture; the appliance uses gentle pressure to gradually widen the arch by applying slight outward pressure on the upper jaw bones. The clinical objective of this treatment is to create a wider arch, which assists in managing crowding and improving bite alignment.

This process, known as distraction osteogenesis, addresses the underlying maxillary bone and provides a skeletal foundation for dental alignment that differs from the tooth-movement-only approach of traditional braces. New bone grows in the gap created by the expansion.

Widening the upper arch also widens the floor of the nasal cavity. This anatomical change can increase the volume of the nasal passage, which may assist in supporting nasal airflow and addressing dental factors associated with mouth breathing.

Bite Resolution

This treatment is commonly used to correct unilateral and bilateral posterior crossbites, including unilateral posterior crossbite cases where the upper teeth sit inside the lower teeth. By widening the arch, it supports more symmetrical jaw development and helps manage the clinical factors associated with uneven growth of the lower jaw.

The Role of Upper Jaw Structure in Dental Alignment

Dental alignment is often influenced by the underlying jaw structure. If the upper arch is narrow, there may be insufficient space for permanent teeth, and arch expansion can help address dental crowding, crossbites, and impacted teeth when lack of jaw width is part of the problem, reducing future orthodontic issues and bite problems as adult teeth come in. By utilizing an arch expander to manage the width of the foundation, we address the skeletal requirements of the mouth. This creates more space for teeth, improves dental alignment, and supports long-term oral health outcomes.

Clinical and Functional Considerations of Palatal Expansion

Support for Nasal Airway Volume

Widening the palate can increase the width of the nasal cavity floor. This anatomical change may assist in supporting nasal airflow and is a clinical factor considered when addressing dental concerns related to mouth breathing or airway volume.

Posterior Crossbite, Bite Alignment and Function

Arch expansion is designed to coordinate the width of the upper and lower jaws. Establishing a more stable bite relationship helps manage the distribution of force across the teeth, which may reduce the risk of uneven tooth wear and address dental factors associated with TMJ function.

Management of Dental Space

Creating skeletal space during early development through early treatment may reduce severe crowding and, in some cases, help prevent tooth extraction later on. By addressing jaw width during growth, our goal is to create sufficient space for permanent teeth to erupt naturally

Structural and Smile Esthetics

Arch expansion addresses the width of the dental arch, which can influence the appearance of the ‘buccal corridors’ (the space visible at the corners of the mouth). This clinical process aims to create a balanced smile that is in proportion with the patient’s underlying skeletal structure and support a more confident smile. By correcting a narrow palate and narrow arches, it can also enhance facial symmetry.

Invisalign® Palatal Expanders vs. Traditional Fixed Appliances

Choosing the appropriate arch expansion protocol is a clinical decision based on the patient’s skeletal maturity and dental development. At our Ottawa clinic, we utilize 3D digital diagnostics to assess your unique anatomy so the orthodontist can choose among treatment options as part of a personalized treatment plan. This detailed imaging allows us to recommend the right expansion device—whether fixed or removable—for maxillary expansion that best aligns with your structural needs and treatment goals. Fixed expanders are cemented onto molars and anchored on the posterior teeth, while removable options are less common and are similar to retainers.

Feature

Invisalign® Clear Expanders

Traditional Fixed Expanders (RPE)

Primary Indication

Phase 1 Early Intervention (Ages 6–10)

Moderate to Severe Skeletal Crossbites

Mechanism

Sequential 3D-printed plastic aligners

Metal rapid palatal expander with an expansion screw (Hyrax, a type of rapid palatal expander)

Activation

Daily tray changes (pre-programmed)

Manual “turns” with a specialized key; patients usually turn the screw daily, and each turn advances to a new hole

Hygiene

High: Removable for brushing/flossing

Moderate: Requires water flosser/proxy brush

Comfort

Smooth biocompatible polymer

Stationary metal; most patients feel pressure or mild soreness rather than pain

Compliance

Requires 20–22 hours of daily wear

Non-removable (guaranteed results)

Traditional fixed expanders use rapid palatal expansion to widen the jaw by about 0.5 mm per day and are often used when skeletal expansion is needed to achieve the desired results.

Adults with developed bones may require surgically assisted rapid palate expansion to address the mid palatal suture, or implant-supported expansion using mini dental implants; in these cases, we work closely with other specialists.

The Journey to a Wider Smile: Our Process

Comprehensive Ottawa Consultation

We evaluate your jaw structure, bite, and health history to see if an arch expander is the right fit for your clinical needs.

Advanced 3D Digital Imaging

No messy molds! We use high-tech iTero scans to design an expander tailored perfectly to your anatomy

Progressive Expansion

Whether using a fixed or removable device, turning the expander’s screw gradually widens the upper jaw in controlled, safe phases within the patient’s mouth. Active expansion often takes about 3 to 6 months to complete, though the full expansion process can range from 2 to 12 months.

Ongoing Local Monitoring

Our team tracks your progress with regular checkups at our convenient Ottawa office to ensure the bone is stabilizing correctly. Once the desired width is reached and the desired expansion is achieved, the appliance is usually left in place for a retention phase of about 6 months to help stabilize the result.

Who Can Benefit from Arch Expanders?

Arch expanders are effective for both children and adults:

  • Children & Teens: Early intervention (Phase 1 Orthodontics) is a form of orthodontic treatment that can guide jaw development. Palatal expansion is commonly used around ages 7 or 8, when a narrow upper jaw is not yet fully fused. Early expansion therapy is often most effective and can help treat severe malocclusions in children and teens, sometimes preventing the need for jaw surgery or braces later.

  • Adults: Thanks to modern Invisalign® technology, many adults can now achieve a broader arch and better bite function discreetly.

The Long-Term Benefits of Arch Expansion

Choosing an arch expander can provide lasting benefits:

  • Improved bite alignment and function

  • Reduced dental crowding

  • Easier oral hygiene and cleaning

  • Enhanced facial symmetry

  • Better airway support and breathing

Frequently Asked Questions (Arch Expander Clinical Guide)

How long does the arch expansion process take?

The active expansion phase typically lasts 3 to 6 months, depending on the required width. However, the appliance must remain in place for an additional 4 to 6 months to allow the newly formed bone in the maxillary suture to calcify and stabilize.

Patients may experience a feeling of “pressure” across the bridge of the nose or cheekbones immediately after an activation (a “turn”). This is a sign the appliance is working. Most patients adapt within 48–72 hours. Over-the-counter anti-inflammatories are usually sufficient for any minor discomfort.

Yes, a temporary gap (known as a diastema) often develops between the two upper front teeth during active expansion. This is a positive clinical sign that the jawbone—not just the teeth—is widening. This gap typically closes naturally or is corrected during the subsequent braces/Invisalign® phase.

While expansion is most predictable in growing children, many adults in Nepean and Ottawa can achieve a broader arch through Invisalign® First or, in more complex cases, Surgically Assisted Rapid Palatal Expansion (SARPE). We assess bone density and periodontal health to determine adult candidacy.

Most expanders are adjusted using a special “key.” We will provide you with a detailed demonstration in the office to ensure you feel confident doing this at home. It’s a simple process that takes only a few seconds.

There is a brief adjustment period. For the first few days, you might notice:

  • Speech: A slight lisp as your tongue learns to move around the appliance.

  • Eating: We recommend softer foods initially. You should avoid sticky or very hard foods (like gum or popcorn) that could loosen the expander.

  • Space: You may notice a gap forming between the two front teeth—this is a great sign! it means the jaw is widening as intended.

Absolutely. If the thought of having an appliance fitted or adjusted causes anxiety, we can discuss mild sedation options like Nitrous Oxide (Laughing Gas) to help you feel completely relaxed during your visit.

Since the appliance is fixed in your mouth, you should brush it every time you brush your teeth. Using a water flosser or a specialized orthodontic rinse can also help clear away food particles from the top of the device.
